Doctor’s Choice Assisted Living is a new concept in assisted living. Our mission is to provide excellent quality assisted living care in an open and warm setting. Our vision is to provide care from the perspective of health care providers with years of experience in geriatrics and who have intimate knowledge of the special needs of seniors. Doctor’s Choice Assisted Living was co-founded by Rachelle Haviland, RN and her husband Ramesh Ramaswamy, MD. Dr. Ramaswamy is a board-certified general internist. He has served as a medical director for Hospice of the Valley and is the current medical director for Banner Home Care. His busy medical practice will be moving to the Doctor’s Choice campus. Rachelle Haviland, RN, BS, a nurse with 14 years of clinical experience in virtually every health care setting, currently manages his office as well as the assisted living.

SERVICES AND CARE:

When we open this fall, Doctor’s Choice Assisted Living will be a fully licensed and accredited assisted living center. All licensed assisted living facilities are required to provide certain services such as laundry, cleaning, meals, and activities. What will set us apart is our superior service in the areas such as assessing health status, medication monitoring and triaging and managing medical urgencies.

DOCTOR’S CHOICE DIFFERENCE:

We know the navigating the healthcare system can be challenging. We will be there to serve as strong advocates and and as a resource for our resident. Imagine having a RN and MD available to assist you or your loved one should the need arise. “After caring for people in the hospital and in our office over the years, we realized there is a real need in the community for a system for seniors in assisted living to have better access to their doctors and better coordination of their care.” Doctor’s Choice Assisted Living allows Dr. Ramaswamy and Rachelle Haviland, RN the ability to be directly involved with the residents in a new and exciting way, and to ensure the highest possible quality and service in the industry. “Our years of experience in healthcare will allow us to streamline the assisted living process for patients.”

We are aiming for September 1, 2008 and we are accepting deposits to hold a space. Please contact us by phone [INSERT HERE] or at welcomehome@doctorschoiceassistedliving.com for more information. What services are provided? All 3 levels of assisted living care are available and customized to the resident wants and needs. How often does Dr. Ramaswamy see the residents? As often as necessary to address their health needs. Do I have to change doctors? No! Dr Ramaswamy’s medical services are an optional convenience for our residents to use. These services are treated like any other doctor’s visit and billed to the patient’s medical insurance. As circumstances vary due to resident wishes and insurance requirements, we look at each resident’s situation individually and create a plan to meet their needs. How much does it cost? The cost of the assisted living services is competitive and tailored to care level. Fees start at $2,500 per month for basic services (room, board, cable TV, and activities). Is respite care available? Yes, short- term care is a service we offer at a per diem (daily) rate. What are phone and cable TV rates? Cable is included in basic rates. Private phone lines are an optional resident expense. Is transportation covered? Basic transportation in Mesa is included. Certain limitations and additional fees may apply depending on travel distance and other factors. Please contact us directly for more details. Do you accept payment from Medicaid or ACCHS? Not at this time.
